Boarding / Daycare Release Form

I, the undersigned, do hereby certify that I am the owner or duly authorized agent for the owner of the animal described on this form.

While vaccinations are not guaranteed to provide 100% immunity, I understand the vaccination equirements and have agreed to them. I also understand that my pet must be free of all internal and external parasites, and that any parasitic treatment will be done at my expense. I understand that in the event of an emergency, that the staff veterinarian, his agents, employees or representatives will use their best judgment in treatment of my pet including the use of sedatives or anesthetics. I do hereby release Kutter Pet Care Center, their agents, employees or representatives from any and all liability while caring for my pet which may include transporting, medical and or emergency treatments.

If requested, the owner agrees to remove the animal within 10 days after request for removal is made. Should the animal not be removed within the specified time, the owner will remain financially responsible for charges incurred, relinquish all claims to the animal, and Kutter Pet Care Center will be at liberty to humanely dispose of the animal as it may see fit.

If I choose to have my dog in daycare, I understand he/she will interact with people and other dogs. With the interaction of dogs, there is always a chance of injury. I assume all risk of injury to my dog. In the event that my dog causes injury to another dog, or to a person while at Kutter Pet Care Center, I agree to indemnify and subrogate Kutter Pet Care Center from any action which may be brought against it, and for any defense, settlement, or judgment entered against it. I will assume all liability for the actions of my dog and agree to maintain personal liability insurance to cover me in the event of such incident.

I agree and understand that in admitting my dog(s), Kutter Pet Care Center has relied on my representation that my dog(s) have not harmed or shown aggression or threatening behavior toward any person or any other dog.

This release and authorization shall remain in effect for this and all future boarding and/or daycare visits.
